VACANCY ANNOUNCEMENT

HOME HEALTH CARE SCHEDULER COORDINATOR: 30 to 40 hours per week / $13.00-$17.00 per hour

Coordinating and maintaining schedule for Home Health Care Plus Clients and Staff, recruitment of new Clients, and Staff.

ESSENTIAL FUNCTIONS


    •Understands and adheres to established Home Health Care Plus policies and procedures.
    •Creating and entering scheduling data (e.g. Quality Assurance, dates and reminders of TB, Performance Appraisals, client admission, client hospital admission and discharge).
    •Maintains staff schedules and supervision, client statistics and reports.
    •Maintain accurate and updated Client files.
    •Schedules client appointments/visits according to care plans and staff availability.
    •Recruitment of new Participants and Home Health Aides.
    •Contacts care providers and Participants regarding day-to-day changes.
    •Discerns client services required as outlined in agreements, urgent requests and care plans.
    •Maintain call-in and retention report.
    •Performs general office duties including but not limited to word processing, photocopies,
    files, shreds, sort/distributes mail, provides reception and telephone services.
    •Attend all general staff meetings, trainings
    •Participate in including but not limited to outside events as assigned; i.e. information tables and literature distribution.


 

LIFTING REQUIREMENTS:

  • Ability to participate in physical activity.
  • Ability to work for extended period of time while sitting, standing and/or being involved in physical activity.
  • Moderate lifting.
  • Ability to do moderate bending, lifting and standing on a regular basis.


 

EDUCATION QUALIFICATIONS:

  • •H.S. Diploma or GED.
    •Knowledgeable and experienced in the healthcare and senior care industries a must.
    •1yr prior scheduling experience preferred. (Experience will be verified)
    •Computer skills including but not limited to MS Office, MS Excel and Scheduling program.
    •Basic medical terminology.
    •Able to work independently, demonstrating sound judgment.
    •Interpersonal, organizational and communication skills.
    •Ability to carry out directives, read and write speak, and understand English as needed for the job.
    •Maturity and ability to deal effectively with the demands of the job.
    •Must have reliable transportation to travel between 3 locations.

PART-TIME RECEPTIONIST: 30 to 35 hours per week / $11.00-$15.00 per hour

Responsible for assisting in coordinating and maintaining the day to day operation of the agency.

ESSENTIAL FUNCTIONS

•Understands and adheres to established Home Health Care Plus policies and procedures.
•Maintain Executive Management scheduled appointments (i.e. calendar and reminders)
•Answers telephone directs calls and take accurate messages using good telephone technique.
•Greets and assists everyone who comes into the office in a professional and courteous manner.
•Data entry of new clients, prospective clients, associates and vendors as needed.
•Sets appointments for new applicants and clients and explains the initial application process.
•Assists with recruiting; i.e. tracking incoming calls, number of applicants, clients and new hires.
•Assists with checking associate work references.
•Photocopies all appropriate identification of new associates.
•Verifies valid licenses and certifications with appropriate licensing body.
•Prepares new hire packets, new client home-care record books, and client information packets.
•Assist with entering scheduling data, creating schedules and verifying schedules.
•Assist with contacting care providers and clients regarding day-to-day changes.

QUALIFICATIONS:

•High School graduate or equivalent.
•Knowledgeable and experienced in the healthcare and senior care industries a must.
•Proven work experience as a Receptionist, Front Office Representative or similar role.
•Basic office and computer skills, and organizational abilities.
•Excellent interpersonal relations abilities.
•Excellent telephone skills.
•Able to work independently, demonstrating sound judgment.
•Ability to carry out directives, read and write, speak, and understand English as needed for the job.
•Maturity and ability to deal effectively with the demands of the job.
•Must have reliable transportation.

HOME HEALTH AIDE: On- Call / $10.00-$12.00 per hour

Provide personal care, meal preparation, light housekeeping and/or shopping and errands to patients in their homes as outlined by the patient's individual Plan of Care. All care / tasks provided are as authorized by the State as acceptable for the level of care in which the employee is functioning and must be detailed on the patient's Aide Care Plan.

ESSENTIAL FUNCTIONS

•Understands and adheres to established Home Health Care Plus policies and procedures.
•Demonstrates a clear understanding of the Mission, Purpose and Philosophy.
•Adheres to ethical business practices by striving to perform in a manner that conforms to the highest standards of ethical behavior, integrity and honesty.
•Assist patients with personal care needs such as bathing, elimination needs, transferring, dressing / undressing, and ambulation together with any household task (e.g., making bed, changing linens, laundry, light housekeeping, etc.) essential to the patient's health in accordance with the patient's individualized care plan.
•Plan / prepare nutritious meals and assist in feeding patients as directed.
•Document services provided in an accurate and timely manner in accordance with agency policy and personal care plan. Always maintain patient confidentiality.
•Measure and record patient's temperature, pulse, respiration and blood pressure when requested and / or directed by the Nursing Supervisor and patient care plan.
•Report any changes in the patient's condition or incidents to Nursing Department in a timely manner.
•Performs tasks / skills as outlined on care plan competently
•Notifies Nursing Department when clarification or education of a task is required.
•Demonstrates appropriate judgment skills to be able to make independent decisions in routine patient care matters.

PHYSICAL REQUIREMENTS:

•Ability to participate in physical activity.
•Ability to work for extended period while sitting, standing and/or being involved
in physical activity.
•Pass a physical examination after being offered employment and a health assessment annually thereafter.
•Possess reliable means of transportation to and from work.
•Able to lift up to 50 pounds, perform one-person transfer under routine and emergency situations, reposition a bedbound patient alone and able to sit, stand, bend stoop, kneel, pull, climb, reach, lift, and perform repetitive movements of the upper extremities
•May be exposed to unsanitary conditions in some home settings.
•May be exposed to high crime areas within the service community.
•Endure exposure to weather and temperature extremes.

QUALIFICATIONS:

•Must be at least 18 years of age.
•High School graduate or equivalent.
•Knowledgeable and experienced in the healthcare and senior care industries a must.
•Successful completion of a training / competency program as prescribed by the Regulatory Agencies of the State in which employed and referenced in the WILLCARE Training Resource Manual and shall possess written evidence of such completion.
•Must be mature, emotionally stable and must display a sympathetic attitude toward providing services for patients at home who are medical problems.
