Home Health Care Plus CDC (HHCP, CDC) is a 501(c)3 non-profit located in West Philadelphia. Our organization focuses on establishing The New West Philadelphia by advocating for sponsored public & private housing, commercial development, and economic development that is in the best interests of our neighborhood. The organization focuses on family self-sufficiency programs and serves as a community resource center to help people get the resources they need to meet basic life's necessities, shelter, utilities, food, and medical care. The organization will also facilitate access to such community services as financial counseling, loans, grants, home repair, and other educational and neighborhood improvement programs.

Civil Action Committee
HHCP, CDC has a community activist group that works with churches, merchants, residents and other community outreach groups helping to organizing the community around citizen participation for neighborhood beautification, voting and knowledge of available programs to strengthening the neighborhood of West Philadelphia.
Helping thy Neighbor Committee
HHCP, CDC works as a liaison between residents and City Officials to facilitate government and resident's participation when soliciting, distributing and managing federal funds, city grants and community sponsorship funding. Contract requirements include:
- Creating and distributing a quarterly newsletter
- Holding quarterly community meetings called The New West Initiative
- Work with the West Philadelphia Block Captains Association
- Maintain a listing of neighborhood organization and their services
- Maintain a listing of housing conditions in the Service area
- Promote City-wide housing programs such as Transitional/Dom Care Housing Program
The New West Beautification Program
The New West Beautification Program was established to mobilize residents in the beautification of West Philadelphia to improve the appearance, including vacant lots. The goals are to make resources and information available to people and assist them in their continuing effort to beautify the neighborhood. Beautification projects are accomplished and unsightly, trash-filled lots become positive fixtures.

HHCP, CDC works with a number of elected officials, community interest groups, non-profits and community Churches to facilitate the development of economic affairs.
The support and participation of community partners include Councilman Curtis Jones, Councilwoman Jannie Blackwell, State Representative Ronald Waters, State Senator Anthony Hardy Williams, State Senator Vincent Hughes, Congressman Robert Brady, Non-profit Achievability, Non-profit 60th Street Business Association, Pastor Dr. James Brown, Mt. Carmel Baptist Church, and business Owner Kevin Parker all have help tremendously in pursuing The New West vision to revive the economy of West Philadelphia.

The New West Revitalization Project
The New West Revitalization Project was created to improve the appearance of the West Philadelphia Corridor, the quality of goods sold, and to reduce the perception of crime with the goal of increasing the number of consumers. The Development Strategy for the New West Revitalization Project outlines the design to accomplish the beautification of the corridor. It also includes exciting renderings of what the Corridor can be with a few key improvements.
HHCP, CDC will become a redevelopment arm of the Economic Development process that will assemble properties. HHCP, CDC will present these assembled parcels to developers interested in creating housing, recreational, retail, and commercial opportunities that are needed in the area.
An effective marketing campaign will retain existing businesses while attracting new businesses, residents, and visitors to the neighborhood. The New West will be marketed through targeted efforts that include promoting historic and cultural assets.

Self-Sufficiency programs help people get the resources they need to meet basic life's necessities, shelter, utilities and food.

Utilities and Heating Assistance
HHCP CDC offers a variety of programs that assist low-income people with their utility and heating costs.
LIHEAP - Low Income Heating Energy Assistance Program (LIHEAP) helps low-income people pay their heating bills.
Crisis is a component of LIHEAP and helps people who have an emergency of being without heat or are immediate danger of not having heat. Emergency situations include running out of heating oil, having the heating gas, electric or water shut off, having a broken furnace.
LIHEAP and Crisis are only available in the winter months, November - March.
To apply, bring the following documentation to HHCP CDC's offices:
1. Proof of income for everyone in the household
- DPA: Green access card or letter from caseworker stating grant amount
- EMPLOYMENT: last 2 pay stubs if paid bi-weekly, last 4 pay stubs if paid weekly
- SOCIAL SECURITY/SSI: print out with current monthly amount received
- UNEMPLOYMENT: determination letter or last 4 stubs
- PENSION: letter stating gross amount and how often you receive
- FAMILY/FRIENDS: letter stating amount and how often you receive assistance/payments; must include name, address, social security number and phone number of the person giving you support
2.Social security cards for everyone in the household
3.The main heating utility bill: ex. If you are applying for electric you must also bring your gas bill or your oil receipt as well as your electric bill. (gas, electric, or oil receipt)
Food Referrals
HHCP, CDC refers low-income people to the Philadelphia Food Hotline. For information on the nearest emergency food bank, call the following number and be ready with a household's zip code.
PA Property Tax or Rent Rebate Program
To file for a rebate you must be a homeowner or a renter.
- Claimant age 65 or older
- Claimant under age 65 with a spouse age 65 or older who resided in the same household
- Widow or widower, age 50 to 64
- Permanently disabled and age 18 to 64
- If you are filing for the first time you must submit the following information
- If you are filing because you are age 65 or older, provide proof of your age.
- If you are under age 65 and your spouse is age 65 or older, prove proof of your spouse's age.
- If you are a widow or widower age 50 to 64, provide proof of your age and a photocopy of your spouse's death certificate.
- If you are permanently disabled, age 18 to 64, you must provide proof of your age and proof of your permanent disability.

- For Social Security disability, Supplemental Security Income (SSI) permanent and total disability, Railroad Retirement permanent and total disability or Black Lung disability, provide a copy of your award letter.
- For Veterans Administration disability, provide a letter from the Veterans Administration stating that you are 100% disabled.
- For Federal Civil Service disability, provide a letter from Civil Service stating that you are 100% disabled.

Community Resource Center
HHCP, CDC goal of the resource center is to serve as a community outreach center to increase the knowledge of education, supportive services, mental health awareness and first aid training, the negative impact of drugs, alcohol and violence on the family and our community.

In partnership with the School District of Philadelphia, HHCP, CDC offers after school programming for students attending neighborhood Elementary, Middle and High Schools in West Philadelphia. Through this local Community Learning Center offers:

Summer Camp
HHCP, CDC offers summer camp for all neighborhood children. Programs include arts and crafts, games, dance, educational programs, and field trips. We also offer career counseling, financial literacy, grace and chivalry workshops and college tours.
